Ask five people what a Semax dose looks like and you'll get five answers, most of them in micrograms. Go back to the actual human studies and something odd happens: the numbers get much bigger. What Is Semax, and Why the Dose Question Dominates

Semax is a synthetic heptapeptide based on a fragment of adrenocorticotropic hormone (ACTH 4–10), with a Pro-Gly-Pro tail added to slow enzymatic breakdown. It was developed in Russian institutes and is registered there as nasal drops — and it appears on Russia's Vital and Essential Drugs list. It is not an approved medicine in Australia, the US, the UK or the EU.

Dose matters more than usual here for two reasons. First, the registered Russian product exists in two strengths — 0.1% and 1% — a tenfold concentration difference, which is fertile ground for confusion once a product is repackaged or reconstituted outside that system. Second, the compound is cleared from plasma quickly and its effects are route-dependent, so a number quoted without a route and a formulation is close to meaningless.

The Doses in the Actual Human Studies

Healthy volunteers: 16 µg/kg, nine men, once

The most-cited human cognition study is Kaplan and colleagues (Neuroscience Research Communications, 1999). Nine healthy men aged 20–40 were tested three times at 7–8 day intervals under a double-blind design. The reported dose was 16 µg/kg intranasally — about 1.1 mg for a 70 kg adult — with improvements described in attention and short-term memory at the start and end of the working day.

Read that honestly: n=9, one group, one laboratory, no replication in the nearly three decades since. It is a signal, not a finding.

Stroke rehabilitation: 6,000 µg per day

The larger human dataset is Russian stroke work. Gusev, Martynov, Kostenko and colleagues (Zh Nevrol Psikhiatr Im S S Korsakova, 2018) reported 110 post-stroke patients given Semax 6,000 µg/day intranasally as two 10-day courses separated by a 20-day interval, alongside rehabilitation, with plasma BDNF, MRC motor scale and Barthel index tracked over roughly five months. Importantly, this was an open-label, non-randomised design — no placebo arm, so BDNF and function changes cannot be cleanly attributed to the peptide rather than to rehabilitation and natural recovery.

That's 6 mg per day. It is roughly an order of magnitude above the low-hundreds-of-micrograms figures that circulate in online nootropic discussion, and those circulating figures are not traceable to any dose-ranging trial.

The citation trail is genuinely messy

The Alzheimer's Drug Discovery Foundation's independent Cognitive Vitality review of Semax summarises the attention and short-term memory literature at doses of 250–1,000 µg/kg — which for a 70 kg adult is 17.5–70 mg — while the Kaplan paper itself reports 16 µg/kg. Those two descriptions of overlapping literature differ by more than an order of magnitude. When secondary sources can't agree on the dose to within a factor of fifteen, no one should be quoting a "standard Semax dose" with confidence.

Why More Isn't Automatically Better

Semax is frequently described in Russian preclinical literature as showing a bell-shaped (inverted-U) dose–response in memory and amnesia models — effects appearing in a middle band and fading at the extremes. If that holds, dose escalation is not a reliable route to more effect, and the "find your dose by going up" heuristic is unsupported.

The flip side showed up in a 2025 qualitative review from University of Texas Rio Grande Valley researchers, presented as a conference poster (not a peer-reviewed paper), which assessed Semax as a Parkinson's neuroprotection candidate. Their reading of the preclinical record: Semax may influence serotonergic and dopaminergic systems, but low doses did not improve motor deficits in animal models, and no clinical trials in Parkinson's have been conducted. Animal-only, unreplicated in humans — worth knowing precisely because it cuts against the marketing.

Route compounds the problem. Semax is degraded rapidly by peptidases, and Russian rodent work has specifically tracked labelled Semax into brain and blood after intranasal dosing — meaning nasal, injected and oral exposure are not interchangeable at the same milligram figure. The ADDF review is blunt that human pharmacokinetics, route and formulation differences, long-term safety and product quality all remain insufficiently characterised.

Semax Australia: Is Semax Legal in Australia?

Here's where dose arithmetic becomes academic for Australian readers. Semax is unscheduled in the Poisons Standard — it has no entry — but that is not the same as being permitted. A single well-designed, placebo-controlled, dose-ranging trial in a defined population, with a pre-registered primary endpoint and published pharmacokinetics for the exact intranasal formulation used. Nothing like that exists in the international registries. Until it does, every Semax dose figure in circulation is an extrapolation from small Russian studies, animal models, or someone else's guess.
